Information Notice

GENERAL EXPLANATIONS:

Pursuant to the Law No. 6698 on the Protection of Personal Data (“KVKK” or the “Law”), natural persons who are defined as data subjects, or their legal representatives (“Applicant” or “Data Subject”), are granted the right to make requests regarding the processing of their personal data as set out in Article 11 of the Law.

In accordance with Article 13, Paragraph 1 of the Law, applications to be submitted to the data controller regarding these rights must be made in writing or by other methods determined by the Personal Data Protection Board (“Board”). Depending on the nature of your request, your application will be answered within thirty days from the date it reaches our company.

APPLICATION METHODS:

Application by Mail, In Person, or via Notary:

The Applicant may submit the duly completed and signed application form along with identity verification documents to the following address in person, via mail, or through a notary public:

Application via Registered Electronic Mail (KEP):

Applications can also be submitted to the address kozmokimya@hs03.kep.tr using a secure electronic signature defined under the Electronic Signature Law No. 5070.

Application via Mobile Signature or Email:

You may also send your application form to info@kozmokimya.com using either:

your registered email address (previously provided to and recorded by the data controller), or
a form signed with mobile signature or electronic signature.

If you submit your request in accordance with the procedures and principles outlined in this form, it will be processed free of charge and concluded as soon as possible, but no later than thirty days, depending on the nature of your request. However, if the transaction requires an additional cost, a fee may be charged in accordance with the tariff determined by the Personal Data Protection Board.

Based on the nature of your request and this form, it is important that the required information and documents are provided to us completely and accurately. If incomplete or incorrect information is provided, this may lead to difficulties in fully and properly conducting investigations related to your request. In such cases, the Company reserves its legal rights. Therefore, this form must be submitted completely, including all required information and documents, depending on the nature of your request.

INFORMATION NOTICE

This application form has been prepared in order to determine your relationship with our Company, to respond to the requests stated in your application, to identify your personal data as the data subject, and to provide an accurate and timely response to your application within the legal period. Our Company reserves the right to request additional information and documents for identity and authorization verification as well as for the evaluation of the application. The information and documents you provide within the scope of your application must be submitted by the authorized person and must be accurate and up to date. In the event of an unauthorized application or if the information provided is determined to be incorrect or outdated, our Company will not accept responsibility and reserves the right to reject the application.

The information and documents specified in and transmitted via this form will be processed by our Company solely for the purpose of evaluating, responding to, and finalizing the application made pursuant to Article 13 of the Law. The information obtained within the scope of this form and application may be collected in written or verbal form, as well as through electronic or physical means. Based on the review conducted within this scope, the relevant information may be shared with the company’s affiliates and third-party service providers such as law firms, solely for the purpose of finalizing this application.

You may exercise your rights under Article 11 of the Law No. 6698 in accordance with the procedures and principles specified in this form.
