EXPLICIT CONSENT STATEMENT UNDER THE LAW ON THE PROTECTION OF PERSONAL DATA

In accordance with the relevant provisions of the Law No. 6698 on the Protection of Personal Data (“Law” or “LPPD”), and within the scope of the General Information Notice on the Protection of Personal Data and the Company’s Personal Data Protection and Processing Policy presented to you by KOZMO KIMYA SANAYI VE DIS TICARET LTD. STI. (“Kozmo Kimya” and/or the “Company”),

I hereby accept and declare that I have been informed by the Company and that I give my explicit consent within the framework of the Law on the Protection of Personal Data, to the processing of my personal data—such as identity information, address information, contact information, and other relevant personal data—either by the Company, acting as the data controller, or by data processors authorized by the Company who take necessary security measures, for the following purposes:

  • Ensuring the best possible provision of the Company’s products and services,

  • Securing the reliable and uninterrupted delivery of such products and services,

  • Maximizing customer satisfaction,

  • Executing payments and related transactions regarding the aforementioned services,

  • Conducting and improving operational processes,

  • Carrying out promotional, marketing, advertising, and campaign activities regarding existing or new products and services,

  • Informing customers about offers, campaigns, and other related services,

  • Fulfilling the requirements of contracts entered into with customers.

In line with the general principles stated in Article 4 of the Law—especially the principle of retaining personal data only for the period required by the relevant legislation or for the purpose for which they are processed—my personal data may be collected, recorded, stored securely in physical or electronic environments, preserved, modified, reorganized, disclosed, transferred, acquired, classified, processed, or their use may be restricted in compliance with the applicable legislation and within the appropriate period for processing purposes.

I hereby acknowledge that I have been informed on the above-mentioned matters and give my explicit consent to the processing of my personal data by the Company in accordance with the LPPD.

I hereby accept and declare that, for the purposes stated above and in accordance with the Law on the Protection of Personal Data, my personal data may be transferred by the Company to the following parties, provided that such transfers remain limited to the purposes mentioned: to the Company’s employees and representatives; to group companies (the Company and/or its business partners and shareholders); to legally authorized public institutions and organizations; to independent audit firms and survey companies within the scope of legal obligations and limitations; and to the Company’s business partners and service providers from which it receives services or cooperates with for the execution of activities and/or services provided to me. I explicitly consent to such data transfers.

Furthermore, pursuant to Article 11 of the Law on the Protection of Personal Data and relevant legislation, I acknowledge that I have the right to apply to the Company and:

  • Learn whether my personal data is being processed,

  • Request information regarding such processing if it has occurred,

  • Learn the purpose of processing and whether my data is used in accordance with such purpose,

  • Learn the third parties to whom my personal data is transferred within the country or abroad,

  • Request correction if my personal data has been processed incompletely or inaccurately,

  • Request deletion or destruction of my personal data in case the reasons requiring the processing cease to exist,

  • Request notification of such correction or deletion to third parties to whom the personal data has been transferred,

  • Object to any result arising to my detriment through the exclusive analysis of my personal data via automated systems,

  • Request compensation if I incur damages due to unlawful processing of my personal data.

I understand that I may exercise these rights by completing the form available at https://kozmokimya.com/data-subject-application-form/ and submitting a signed copy along with identity-confirming documents either:

  • In person to the Company’s Headquarters at:

  • Or via notary public,

  • Or through other methods defined under the Law on the Protection of Personal Data.

Additionally, I confirm that the personal data I have shared with the Company is accurate and up to date; and I undertake to inform the Company of any changes to such information.

I hereby give my explicit consent to the processing, use, and sharing—limited to the purpose of processing—of my personal data, including sensitive personal data as defined by the Law on the Protection of Personal Data. I also consent to the storage of such data for the required period and acknowledge that I have been duly informed in this regard.
